Follow these simple steps to complete your Level 2 fingerprint background check requirement in Jacksonville FL:

1) Obtain an ORI number from the Florida Business & Professional Regulation or visit: http://www.myfloridalicense.com/dbpr/servop/testing/documents/finger_faq.pdf for listing of ORI numbers.

2) Bring in your Electronic Fingerprint Form with the ORI number along with your current (not expired) driver’s license, government issued identification card, or passport to First Coast LIVE SCAN at 3728 Philips Highway, St. 13, Jacksonville, FL 32207. Please call us at (904) 778-5547 if you have any questions.

3) Your fingerprint are captured by First Coast LIVE SCAN then sent to the Florida Department of Law Enforcement (FDLE) for processing.

4) First Coast LIVE SCAN will provide you with a Transaction Control Number (TCN) report that verifies your live scan submission has been processed. The TCN number can be given to the company that is requesting the background check as it will help them track results.
